Special Conditions for Beauty Center and Healthcare Center Premises City Council Shah Alam
1. Premises that can be licensed are:-
- hotels (rated 3 stars and above),
- club house,
- business complex/shopping complex,
- shop office/shop house.
- Health Care Center activities are only allowed on the ground floor of the premises.
2. Must submit a list of employees on the premises.
3. Submit a copy of valid Passport and Travel Visa for Foreign workers (Passport and visa must be verified by the Malaysian Immigration Department).
4. The licensee is not allowed to make any partition either permanent (concrete) or semi-permanent (gypsum board).
5. The design of beauty and health care centers should follow an open concept and must have perfect and sufficient light and ventilation.
6. The entrance to the premises must be a transparent mirror for a quarter of its surface and the door must not be locked during business hours.
7. Employees who provide massage treatment must pass a health examination by a medical officer every six months.
8. It is not allowed to hire workers/masseurs under the age of 21.
9. Employees must wear uniforms that are clean, decent and not eyes flashy.
10. A register book containing names, no. ID card/passport, residential address, photograph and details related to employees and massage therapists must be provided and always updated.
11. The licensee must display signs that are easy to see for employees and customers to follow, which are:-
a) No smoking
b) It is forbidden to drink alcohol
c) Gambling is prohibited.
12. It is forbidden to dispose of biological waste without the permission of the Council.
13. The deposit for the Beauty Center license is RM 1,000 AND RM 2,000 for the Health Care Center.
