Special Requirements for Applications for Premises Involving Food (Restaurant / Kindergarten / Nursery)
1. List of employees' names, copies of valid Identity Cards, Passports and Visas for foreigners for Food Supervision premises.
2. A copy of the Food Handling Card (Had a Doctor's Examination and TY2 Injections) for the Food Supervision premises.
3. Pictures of Oil or Grease Traps in Sinks and Food Preparation Equipment Washing Areas.
4. Picture of the Exhaust Fan (Exhaust Fan) on the premises.
5. Copy of the Letter of Appointment of the Pest Control and Disease Carrier Company (Pest Control).
6. Must register as a food premises operator at the Ministry of Health Malaysia by submitting the completed Form KKm-PPKM-1/09.
7. For Cafeteria activities on the Upper Floor:-
i. Only heating, baking or steaming activities are allowed using electrical equipment for the preparation of food such as bakery (cake/bread) and pastry (biscuit/confectionery)
ii. Need to get a supportive review from the owner / tenant of the premises on the Ground and Upper Floors
iii. Keeping clean
iv. No nuisance complaints from members of the public nearby / adjacent premises
v. Approval to license cafeteria activities on the Upper Floor is subject to comments / support from the Fire and Rescue Department
